GXO offers supplemental insurance through MetLife that helps with out-of-pocket expenses not covered by health insurance. Each program pays cash benefits directly to you to be used however you choose. You can enroll in Accident Insurance, Hospital Indemnity Insurance and/or Critical Illness Insurance during Open Enrollment. If you elect coverage, you pay for coverage through convenient payroll deductions.

Accident Insurance
Accidents can happen in an instant, affecting you or a loved one. MetLife’s Accident Insurance plan can help cover expenses such as:
- Ambulance rides
- Emergency room visits
- Surgery and anesthesia
- Prescriptions
- Major diagnostic testing
- Burns, and more
- You can choose a low or high option, as shown below.
Hospital Indemnity Insurance
Your HSA or PPO plan may only pay a portion of your entire hospital stay. The MetLife Hospital Indemnity plan provides financial assistance to enhance your current coverage. Benefits are payable for a hospital admission and confinement for a covered accidental injury or covered sickness (including pregnancy). You can choose a low or high option, as shown below.

Critical Illness Insurance
The MetLife Critical Illness plan provides additional financial resources to help with medical costs or ongoing living expenses. Coverage is available for you, your spouse and your dependent children. Guaranteed Issue (GI) amounts that don’t require Evidence of Insurability (EOI) are:
- $20,000 employee/$10,000 dependent
- $10,000 employee/$5,000 dependent
The plan pays a lump-sum benefit for the initial diagnosis for covered illnesses such as:
- Cancer
- Heart attack
- Stroke
- Kidney failure (end-stage renal failure)
- Major organ transplant
- Coronary artery bypass surgery
Health Screening Reward NEW
Health screenings are an important part of managing your health — they can help find problems early, when they may be easier to treat. Now, there’s even more reason to take action. You and your dependents can earn $50 after receiving a covered health screening from your provider, such as blood tests to determine cholesterol, mammograms, colonoscopies and more.
After receiving a covered screening, submit your claim with your provider’s name, phone number and address, along with the date it was completed. You’ll receive a check in the mail within a few business days of the claim being processed — no hard copy proof required. It’s that easy!
