Hello Heart NEW

We’re partnering with Hello Heart to provide a free monitoring kit that makes it simple for you to track your heart health and understand what’s going on — all from the privacy and comfort of your own phone. Hello Heart is offered at no cost to Anthem members (age 18 and older) and includes a blood pressure monitor that connects to an engaging app. You can worry less, plus it’s rewarding and fun! This program is fully covered if you are enrolled in an Anthem medical plan and meet the medical conditions required for support. For more information, review this flyer.

Hello Heart can help you embrace healthier behavior, which can reduce the risks of high blood pressure, high cholesterol and heart disease. It also helps users catch blood pressure readings that are extremely high, as well as irregular heartbeat, encouraging you to talk to your doctor to identify potential risks in time.

Improving outcomes with “Medications”

Hello Heart’s “Medications” is a breakthrough feature that helps you understand how your medication may be influencing your heart health, while making it fun and easy to build the habit of taking medication on time. Connecting the dots in this way puts you in the driver’s seat, empowering you with insights you can use to build lasting healthy habits — like medication adherence — while also identifying potential risk in drug interactions, overmedication and undermedication. These features can help improve health outcomes and reduce healthcare costs.

Supporting women’s needs

Hello Heart provides personalized digital coaching for women, and tools to empower them to track, understand and manage their heart health including:

  • Heart attack symptoms — Tailored user flows that reflect men’s and women’s differing heart attack symptoms
  • Menopause — Dedicated features that provide an education on its impact on hearth health, actionable insights focused on managing symptoms during menopause and reducing heart risk
  • Clinician report — Empowers women to advocate for themselves in the health care system by providing shareable physician reports of their tracked health information to facilitate better conversations with doctors
  • Cholesterol — Easy-to-understand, personalized explanations of cholesterol test results, clarifying the complex results and accounting for the varying normal ranges for men and women

See Avoid the tobacco surcharge below.

Anthem

Anthem’s Tobacco Free program can help you and your covered family members stop using tobacco products. If you are enrolled in a GXO medical plan, Tobacco Free is provided at no cost to you.

Anthem’s Tobacco Free program:

  • Is designed with five visits
  • Addresses all forms of tobacco use, including smoking, chewing and vaping
  • Provides video visits with health coaches for behavior modification coaching and support
  • Offers Nicotine Replacement Therapy, which can be ordered by a health coach and sent directly to your home
  • Can arrange an optional physician office visit (at your expense based on your GXO medical coverage) should a prescription be needed as part of treatment
  • Provides access to health coaches by appointment seven days a week in all 50 states

You can also get one-on-one support to help overcome any emotional, physical or social barriers that might stand in the way of achieving lasting success.

Kaiser HMO

Review this quit smoking flyer or visit kp.org/quitsmoking for more information about Kaiser Permanente’s tobacco cessation program.

Avoid the tobacco surcharge

If you and/or your covered spouse complete Anthem’s Tobacco Free program, Anthem will notify the GXO Benefit Center and your tobacco surcharge will be removed for the remainder of the plan year. If you complete a tobacco cessation program offered by Kaiser Permanente or an alternate vendor (preapproved by the GXO Benefit Center), you will need to submit a certificate of completion to the GXO Benefit Center to have the tobacco surcharge removed. Once the surcharge has been removed, your tobacco-free status will roll over automatically from year to year.

LiveHealth Online

When you are enrolled in an Anthem GXO medical plan, you have access to tools and resources that can save you time and money, including a telemedicine service through Anthem: LiveHealth Online. This resource connects you directly to a board-certified doctor who can discuss your health care concerns, review treatment options and even prescribe medication over the phone or virtually. It’s like visiting the doctor without the travel time and waiting!

Televisits with LiveHealth Online usually cost less than going to a convenience care or urgent care clinic, and significantly less than going to an emergency room. And your out-of-pocket cost is the same or less than a visit with your primary care provider.

Remember, you should only use telehealth services for non-life-threatening conditions.

Register for one or both today, so you’ll be ready to use a telehealth service when and where you need it.

Case management

If you or a family member is faced with a complex health condition, Anthem case managers can help.

Case management teams include specially trained nurses and other licensed health care professionals who have the skills and experience necessary to provide compassionate assistance to people with medical or mental health needs. Anthem’s case managers are supported by the company’s nationwide staff of board-certified physicians and are experienced in both medicine and medical insurance.

Anthem case managers are trained to coordinate access to care (for example, at home, in the hospital or as an outpatient), explore service alternatives, monitor progress, assist with discharge planning arrangements and follow-up, and help ensure that health insurance benefits are used effectively.

Examples of Anthem case management programs include:

  • High-Risk Maternity
  • Oncology (cancer)
  • LIFESOURCE/Transplants
  • Catastrophic
  • Depression
  • Disability
